The patient, who had previously under rgone mitral valve replacement with B ioprosthetic valve in 2013, and a recen techo confirmed degeneration of that prosthesis with severe narrowing and leak along with very high right heart p ressure (severe pulmonary hypertens ion) A second operation to replace this will be prohibitively higher risk, hence he was planned to have elective non -su rgical Transcatheter Mitral ViV replace ment in end of November, however, his journey took an unexpected tur n when he suffered a fall at home, leading to a left leg fracture neck of femu r (broken hip bone).

The urgency of his hip condition and t he critical nature of his heart ailment prese nted a daunting challenge. It was a highr isk procedure that required a multimedi a multidisciplinary approach and the onl y safer way to treat him is to do both pro cedures under the same general an aest thesia, first the Transcatheter Mitral Vi V followed by Left Total Hip Replaceme nt, in an Hybrid Cath Lab Operating Roo m. This is available in very few centres i n India, and given the availability of the Hybrid Cath Lab OR, Cardiac and Ortho pedic expertise, this patie nt underwent Hybrid procedure safely and was mobili zed within 36 hours.

“The patient underwent a transcatheter mitral valve-in-valve procedure through groin access, a minimally invasive techni que that immediately reduce d his right-side pressure from 130 to 60 and a norm ally functioning mitral valve. With his h eart condition stabilized, the orthopedic team seamlessly proceeded with the hip replacement surgery on the same opera ting table at the Hybrid Operating Roo m,” said Dr R Anant haraman , Senior Int erventional Cardiologist Kauvery Hospi tal Chennai.

The procedures were a resounding suc cess. The patient was extubated from the ventilator and mobilized within 36 hours, defying all odds. His swift recov ery enabled him to return to his villag e and resume his active lifestyle.
